What is the proper industry standard for slating and naming your audition file?

Just say the word "Slate" and your name. That's it. That's all they need. Good luck, but if your name is Slate, it will confuse them big time.

Actually, all you need to say is your name, then pause before beginning your read. If you are reading for a commercial or animated project and there is more than one role, you can say your name, the role, pause and begin. If you are doing two takes, you can say, your name, "two takes", pause, begin your read then "take two" before the second take.
