Dr Smith notes that a genital vesicular ulcerative rash is pretty commonly genital herpes, although other conditions may resemble genital herpes and should be considered in a differential diagnosis. Most notably, chancroid and syphilis need to be distinguished in the differential, but they’re usually a bit more obvious in terms of clinical presentation. Chancroid can manifest as painful, tender, nonindurated lesions with a serpiginous border surrounding a friable base covered with gray/yellow, necrotic, purulent exudates. Half of all cases have painful, unilateral inguinal adenitis. Primary syphilis can consist of one or more painless, indurated ulcers. Noninfectious conditions to consider in the differential diagnosis of genital herpes include Crohn's disease, Behçet's syndrome, trauma, contact dermatitis, erythema multiforme, Reiter's syndrome, psoriasis, and lichen planus.
