Blue Cross and Blue Shield of Minnesota is offering you and your cover dependence the following vision care plan, comprehensive eye examination covered under your health plan and one pair of eyeglasses or contact lenses in lieu of eyeglasses annually. A $10 co payment will apply to your eyeglass lenses. Included. Eyeglass lens types are single vision bifocal. Try focal, lenticular and oversized lenses in any available prescription range. Additional popular enhancements are included or available at fixed member out of pocket costs, which are significantly below retail price. Summary features for eyeglasses include access to the Davis Vision, Exclusive collection of fashion designer and premier frames. All frames under this collection are covered in full when selecting a frame that is not a part of the Davis Vision Exclusive collection of frames. You will receive a $150 allowance and a 20% discount will be applied to the amount over $150. At a Vision works location, you will receive a $200 allowance and an additional 20% discount on the amount over $200 when selecting eyeglasses. Your benefit includes a one year breakage warranty covering, repair or replacement of your plan covered eyeglasses, including lenses and frames. Private practice providers may prescribe you contact lenses from the Davis Vision Exclusive collection of contact lenses. This collection includes evaluation fitting and are covered in full for all other types of contact lenses from the providers supply. You will receive a $150 allowance, and a 15% discount will be applied to the amount over $150. When selecting contact lenses from the providers supply, you will receive a $60 allowance and a 15% discount toward the cost of the evaluation and or fitting a $10 copay will apply. Please note that the additional discounts may not be available at all participating retail locations. Visually required contact lenses, also known as medically necessary contacts, will be covered in full with prior approval. The Davis Vision program provides access to private practice providers and retail chain optometric locations, or the option to shop online by using your benefits at one of our online vendors. Vendors such as befitting dot com, 1 800 contacts dot com and glasses dot com. The Davis Vision Exclusive collection of frames and contact lenses, as mentioned earlier, are available at most private practice office locations.
