the RN case manager presents the history and examination findings at the time of admission. This presentation should include information that answers the question. Why Hospice? Why now? That supports a prognosis of six months or less. This should be projected on the screen for the team to view during the presentation. In addition to the primary hospice diagnosis, any significant conditions secondary to the primary diagnosis and any significant co morbid diagnosis should be discussed. All conditions that are actively contributing to the terminal prognosis are considered related, and medications for these conditions need to be marked in the record as covered by the hospice provider. Although not included in this presentation, social work, spiritually care volunteer coordinator and bereavement coordinator are invited to participate in the discussion of each admission as well.
